              LONG TERM CARE SPECIALIST

LONG TERM CARE SPECIALIST  is needed for the Department of Senior Services & Community Transit with salary of $17.13 per hour. Provide options counseling, Medicaid Waiver information, and referrals for community based programs for institutionalized individuals 50 + years.  Provide application assistance and transitional case management services to support seniors and disabled adults toward safely moving from the nursing home environment into the community.This position works approximately 25 hours per week. It is financed, and contractual under  the Money Follows the Person Program Grant and renewable based upon continued funding. 

Position requirements: Bachelors Degree in Gerontology, Human Services, Business Administration, or related field. Three (3) or more years working with a long-term care population or in Human Service program coordination, including equivalent technical training, education will be considered. Applicant must have a valid Driver's License.

Duties: Include, but not limited to, providing program education and advocacy for County residents within Assisted Living facilities, maintain knowledge of applicable local, State, and Federal laws and regulations; receive and respond to, and refer complaints regarding abuse, neglect, and/or violations of care to appropriate agencies, maintain Ombudsman and Public Guardianship files, compile information from adult care facilities regarding long term care, conduct site visits to nursing home facilities to ensure needs are met; represent the agency at meetings and the Public Guardianship Review Board; performs all duties as assigned.
